Create the list of requested surveys that your squadron will take. For help, call: 888-603-3170.

Use the controls at the bottom of this page for the following steps:

Step 1. Select desired surveys

NOTE: USMC squadrons requesting GROUND safety surveys (e.g., PMV, MTRCYCL, HHQ, D&D, OD&R)
Go to the USMC Ground Climate Assessment Survey System (GCASS) at: https://SemperFiSurveys.org

Step 2. Enter the maximum number of people who can take the survey.
Step 3. Description of group (for most, this will be "All Aircrew" or "All Maintainers" but could be specific like "Instructors", "Students", "Rotary Wing", etc.)
Step 4. Select "Add to Request List" for each survey needed. When ALL surveys have been added to the list, select the long green button below the list of surveys.
Step 5. If you wish to add a couple additional survey items to the end of your survey(s), you may see a list of possible items at this link. Then, speak to the survey managers (888-603-3170) to implement the additional items.
